Event Coverage Request Form

Please utilize the form, below, to request coverage of your Event.  Details about who would be responsible for issuing Passes or access, if required, should be included in the Comments section.  Also, included hyper-links in the Comments section, if this Event has a Web address or Internet location. 

When providing dates in the Start and End field, use the following format: January 23, 2012.  Times, at least at this stage, are not critical.  We can always get any additional information when one of our representatives contacts you.

To ensure your Event will be covered, we must receive notification at least one week prior to the Event's Start date and it will be at the sole discretion of MainLineBiz.com whether or not the Event will be covered, in part, in its entirety, or at all.

Any material provided to the Event organizers developed by MainLineBiz.com, its agents or representative will be at no charge*.

*Some restrictions apply, so please speak with an authorized Representative and let them know, prior to the Event, of any special needs or requiremnet.
